Student Athlete Athletic Training Forms

• Student-Athletes must be added to the official roster by the coaching staff of their respective team.
  • Those additions are sent to the Director of Compliance - Jen Enke (enkejz@alfredstate.edu or 607-587-4383). Any questions about eligibility/compliance can be directed to Jen Enke 

• All incoming student athletes need a physical within six months of the first practice. All physicals must be on our Alfred State physical form. Freshmen/Transfer Physical Form
• All other information needed will be sent to student-athletes by their respective athletic trainer or coach prior to start of practices.
